ABSTRACT

Introduction. The intake of energy-dense unhealthy food at school could influence the overall energy intake of children. Objective: To characterize the patterns of buying a snack at school and to analyze the association of these patterns with the source of the school snack (brought from home or bought at school) and screen time. Materials and methods: A cross-sectional study was conducted from October 2018 until May 2019 among school children (9 to 12-year-olds) from Cuenca-Ecuador. Intake of energy-dense unhealthy food groups, the source of the school snack, and screen time were assessed using questionnaires. Multiple correspondence analysis was used to identify intake patterns of unhealthy food groups, and logistic regression to assess the association between intake of energy-dense unhealthy food groups (FG) with the school snack source. Results: Among 1,028 children recruited 63%, 42%, 30% and 22% of the children reported consuming sweets/confiture, savory snacks, fast food, and pastry products, respectively, at least two days per week. Around 39% of the children were classified as "Frequent consumers of energy-dense unhealthy FG". Buying the snack at school was associated with a more frequent intake of sweets/confiture (OR=1.56, CI 95% =1.05-2.32) and fast food (OR=2.01, CI 95% =1.15-3.50) during the week, as well as with being classified in the "frequent consumer of energy-dense unhealthy FG" (OR=1.99, 95% CI=1.40-2.82). Conclusions: Ecuadorian children still consume prohibited unhealthy foods at school. These results reinforce the importance of evaluating, monitoring, and adapting strategies to promote a balanced diet(AU)


Introducción. La ingesta de alimentos poco saludables con alto contenido energético en la escuela podría influir en la ingesta energética total de los niños. Objetivo: Caracterizar los patrones de compra de snacks en el colegio y analizar la asociación de estos patrones con la procedencia del snack escolar (de la casa o comprada en el colegio) y el tiempo de pantalla. Materiales y métodos: Se realizó un estudio transversal (octubre 2018-mayo 2019) en escolares (9-12 años) Cuenca- Ecuador. La ingesta de grupos de alimentos (FG) no saludables altos en energía, la fuente del snack escolar y el tiempo de pantalla se evaluaron mediante cuestionarios. Se utilizó análisis de correspondencias múltiples para identificar patrones de ingesta de FG poco saludables y la regresión logística para evaluar asociación entre la ingesta FG no saludables con la fuente del snack escolar. Resultados: En 1.028 niños 63%, 42%, 30% y 22% declararon consumir dulces/golosinas, aperitivos salados, comida rápida y productos de pastelería, respectivamente, al menos dos/días/ semana. Un 39% fueron clasificados como "Consumidores frecuentes de FG poco saludables de alta densidad energética". La compra de los snacks en el colegio se asoció con una ingesta más frecuente de dulces/golosinas (OR=1,56; IC95%=1,05-2,32) y comida rápida (OR=2,01; IC95%=1,15-3,50) durante la semana y con la clasificación en el grupo" consumidores frecuentes de FG poco saludables de alta densidad energética" (OR=1,99; IC95%=1,40- 2,82). Conclusiones: Los niños ecuatorianos aún consumen alimentos no saludables prohibidos en la escuela. Estos resultados refuerzan la importancia de evaluar, monitorear y adaptar estrategias para promover una dieta balanceada(AU)

ABSTRACT

Introducción: La esclerosis sistémica es una enfermedad de etiología desconocida crónica, caracterizada por fibrosis tisular, disfunción vascular y autoinmunidad. Entre otros síntomas, las alteraciones gastrointestinales son responsables del riesgo de malnutrición. De manera que la evaluación de la composición corporal es fundamental. Objetivo: Caracterizar la composición corporal de los pacientes con esclerosis sistémica. Métodos: Se realizó un estudio descriptivo y transversal de pacientes que se atienden en la consulta protocolizada del Servicio de Reumatología del Hospital "Hermanos Ameijeiras". La muestra quedó conformada por 90 pacientes con diagnóstico confirmado de esclerosis sistémica, atendidos entre los años de 2019 y 2021. Resultados: Dentro de las manifestaciones clínicas, predominó en estos pacientes la forma difusa. Se observó que a mayor tiempo de la evolución se presentaban más alteraciones gastrointestinales y úlceras digitales. Más de la mitad de los pacientes tenían sobrepeso u obesidad y se encontraron valores altos y muy altos de grasa corporal. A mayor índice de masa corporal, mayores valores de grasa corporal, grasa visceral y menor masa magra. Los pacientes con bajo índice de masa corporal tenían una tendencia significativa a presentar alteraciones gastrointestinales y no se evidenciaron diferencias en los porcentajes de grasa visceral ni de grasa corporal sin embargo, el porciento de la masa magra fue significativamente inferior. Conclusiones: Se caracterizó la identificación temprana y detallada de esta enfermedad para disminuir la mortalidad y mejorar el pronóstico de estos pacientes, mediante el análisis de la composición corporal.


Introduction: Systemic sclerosis is a chronic disease of unknown etiology, characterized by tissue fibrosis, vascular dysfunction and autoimmunity. Among other symptoms, gastrointestinal disturbances are responsible for the risk of malnutrition. In this sense, the evaluation of body composition is fundamental. Objective: To characterize the body composition of patients with systemic sclerosis. Methods: A descriptive and cross-sectional study was carried out in patients assisted in the protocolized consultation of the Rheumatology Service of Hospital Hermanos Ameijeiras. The sample was made up of 90 patients with confirmed diagnosis of systemic sclerosis who were treated from 2019 to 2021. Results: Diffuse form predominated in these patients as the clinical manifestations. It was observed that the longer the evolution, the more gastrointestinal alterations and digital ulcers were presented. More than half of the patients were overweight or obese, as high and very high values of body fat were found. The higher the body mass index, the higher the values of body fat, visceral fat and lower lean mass. Patients with low body mass index had significant tendency to present gastrointestinal alterations and no differences were found in the percentages of visceral fat or body fat. However, the percentage of lean mass was significantly lower. Conclusions: The early and detailed identification of this disease was characterized to reduce mortality and improve the prognosis of these patients through the analysis of body composition.

La toxoplasmosis es una patología parasitaria que es producida por la presencia de un protozoo a nivel intracelular. Tiene varias consecuencias tanto en adultos como en niños, sin embargo, en estos últimos se han evidenciado secuelas como las discapacidades. Objetivo. Realizar una revisión de información de la intervención terapéutica en niños con secuelas de toxoplasmosis. Metodología. Se realizó una revisión bibliográfica en buscadores académicos incluyendo los descriptores: discapacidad, discapacidad física, sensorial e intelectual, intervención psicoterapéutica, intervención terapéutica, toxoplasmosis y toxoplasmosis congénita. Se logró compilar información y se delimitó las secuelas de la toxoplasmosis en dos tipos de discapacidad: intelectual y sensorial. Dentro de esta última, se encontró la prevalencia de las deficiencias a nivel auditivo y visual como consecuencias. Resultados. Se brindó una definición de cada término, así como también la importancia de mantener una perspectiva de derechos e inclusiva al momento de realizar una intervención Psicológica. Conclusiones. A nivel de la Psicología Clínica se identificó que no existe un tratamiento específicamente para las secuelas de toxoplasmosis, sin embargo, el objetivo terapéutico gira en torno a las necesidades individuales de cada sujeto, en este caso, sobre las discapacidades de la población infantil.


Toxoplasmosis is a parasitic pathology that is caused by the presence of a protozoan at the intracellular level. It has several consequences in both adults and children, however, sequelae such as disabilities have been seen in the latter. Objective. To carry out a review of information on therapeutic intervention in children with sequelae of toxoplasmosis. Methodology. A bibliographic review was carried out in academic search engines including the descriptors: disability, physical, sensory and intellectual disability, psychotherapeutic intervention, therapeutic intervention, toxoplasmosis and congenital toxoplasmosis. Information was compiled and the consequences of toxoplasmosis were defined in two types of disability: intellectual and sensory. Within the latter, the prevalence of deficiencies at the auditory and visual level as consequences was found. Results. A definition of each term was provided, as well as the importance of maintaining a rights and inclusive perspective when carrying out a psychological intervention. Conclusions. At the level of Clinical Psychology, it was identified that there is no treatment specifically for the sequelae of toxoplasmosis, however, the therapeutic objective revolves around the individual needs of each subject, in this case, on the disabilities of the child population.


A toxoplasmose é uma patologia parasitária causada pela presença de um protozoário no nível intracelular. Tem várias consequências tanto em adultos como em crianças, no entanto, sequelas como deficiências têm sido observadas nestes últimos. Objetivo. Realizar uma revisão das informações sobre a intervenção terapêutica em crianças com sequelas de toxoplasmose. Metodologia. Foi realizada uma revisão bibliográfica em buscadores acadêmicos incluindo os descritores: deficiência, deficiência física, sensorial e intelectual, intervenção psicoterapêutica, intervenção terapêutica, toxoplasmose e toxoplasmose congênita. As informações foram compiladas e as consequências da toxoplasmose foram definidas em dois tipos de deficiência: intelectual e sensorial. Dentro deste último, encontrou-se a prevalência de deficiências a nível auditivo e visual como consequências. Resultados. Foi fornecida uma definição de cada termo, bem como a importância de manter uma perspectiva de direitos e inclusiva na realização de uma intervenção psicológica. Conclusões. Ao nível da Psicologia Clínica, identificou-se que não existe um tratamento específico para as sequelas da toxoplasmose, contudo, o objetivo terapêutico gira em torno das necessidades individuais de cada sujeito, neste caso, sobre as deficiências da população infantil.

ABSTRACT

La enfermedad de Rendu-Osler-Weber, también conocido como telangiectasia hemorrágica hereditaria, es una enfermedad genética de herencia autosómica dominante con penetrancia incompleta. Afecta por igual a ambos sexos y los síntomas se inician habitualmente entre los 20 y 40 años, pero se considera que la enfermedad está infradiagnosticada. Típicamente las formas clínicas y el debut de esta enfermedad se asocian a los órganos y tejidos que se afectan con mayor frecuencia: telangiectasias en mucosas y en piel, epistaxis, sangramiento gastrointestinal, pulmonar e intracerebral. En contraste, el caso clínico que se presenta se caracteriza porque las primeras manifestaciones clínicas que motivaron la consulta médica fueron crisis de dolores e inflamación ósea en el miembro superior derecho, lo cual es inusual y se inscribe como un elemento de novedad en la enfermedad. Es el objetivo de esta publicación exponer un caso de telangiectasia hemorrágica hereditaria con una forma de presentación atípica en una adolescente. Al alta hospitalaria, la paciente estaba estable, sin complicaciones. Se recomendó seguimiento hospitalario fundamentalmente por la especialidad de Neumología, por ser los pulmones los órganos más afectados(AU)


Rendu-Osler-Weber´s disease, also known as hereditary hemorrhagic telangiectasia, is a genetic disease of autosomal dominant inheritance with incomplete penetrance. It affects both sexes equally and symptoms usually begin between the ages of 20 and 40, but it is considered that the disease is underdiagnosed. Typically, the clinical forms and the onset of this disease are associated with the organs and tissues that are most frequently affected: mucosal and skin telangiectasias, epistaxis, gastrointestinal, pulmonary and intracerebral bleeding. In contrast, the clinical case that is presented is characterized because the first clinical manifestations that motivated the medical consultation were crises of pain and bone inflammation in the right upper limb, which is unusual and is inscribed as an element of novelty in the disease. The objective of this publication is to present a case of hereditary hemorrhagic telangiectasia with an atypical presentation in a female teenager(AU)

Abstract Objective The present article presents a literature review concerning the microbiota of breast milk and the influence of epigenetics in the susceptibility to COVID-19. Methods A literature review. Results Breastfeeding transfers microbiota, nutrients, diverse white blood cells, prebiotics, hormones, and antibodies to the baby, which provide short- and longterm immunological protection against several infectious, gastrointestinal, and respiratory illnesses. The little evidence available shows that breast milk very rarely carries the SARS-CoV-2 virus, and even in those cases, it has been discarded as the source of contagion. Conclusion The reviewed studies show evidence of a beneficial effect of breastfeeding and highlights its importance on the current pandemic due to the immune reinforcement that it provides. Breastfed individuals showed better clinical response due to the influence on the microbiota and to the nutritional and immune contribution provided by breast milk, compared with those who were not breastfed.


Resumo Objetivo O presente artigo apresenta uma revisão de literatura sobre amicrobiota do leite materno e a influência da epigenética na suscetibilidade à COVID-19. Métodos Revisão de literatura. Resultados A amamentação transfere microbiota, nutrientes, diversos glóbulos brancos, prebióticos, hormônios e anticorpos para o bebê, os quais proporcionam proteção imunológica de curto e longo prazo contra diversas doenças infecciosas, gastrointestinais e respiratórias. As poucas evidências disponíveis mostram que o leite materno transportamuito raramente o vírus SARS-CoV-2, emesmo nestes casos, ele foi descartado como fonte de contágio. Conclusão Os estudos revisados mostram evidências de um efeito benéfico da amamentação e destacam sua importância na atual pandemia devido ao reforço imunológico que ela proporciona. Os indivíduos amamentados mostraram melhor resposta clínica devido à influência sobre a microbiota, e à contribuição nutricional e imunológica proporcionada pelo leite materno, em comparação com aqueles que não o foram.

ABSTRACT

Korsakoff's syndrome (KS) is an insidious and progressive neuropsychiatric disorder that affects specific neurocognitive functioning, especially in tasks that require sustained attention, memory, executive functions, and visuospatial functioning. Usually, this disease generates neuropsychiatric complications that worsen the quality of life (QOL) of patients in the medium term. We present a case of a 63-year-old male who presented with a diagnosis of advanced Korsakoff's syndrome and has a clinical history of recurrent memory loss and a history of alcohol abuse. The patient showed difficulty manipulating immediate information, associated with a possible frontal lobe dysfunction, and inability to remember material given through the auditory pathway. The patient showed a psychiatric clinical picture which is constantly worsening his and his immediate caregiver's QOL. The data obtained demonstrate that the patient presents a progressive cognitive impairment, which in the short term is correlated with Korsakoff-type dementia. It is suggested to carry out functional neurorehabilitation plans aimed at improving the QOL of the patient, his immediate caregiver, and future people with this type of diagnosis.
